Eight-year-old Jackson Millarker stars in the episode as Tom, a transgender friend of Lily, the daughter of Cam (Eric Stonestreet) and Mitchell (Jesse Tyler Ferguson), NewNowNext reports. As Variety points out, transgender actors have appeared on other television shows including Orange Is the New Black and Nashville, but they've all been adults.
